Can’t Keep Your Appointment?
Please notify us at least 24 hours in advance of your scheduled appointment if you know that you will not be able to come. This will allow us to schedule patients with urgent need of a visit. Cancellations with less than 24 hours’ notice may result in a cancellation fee of $50 charged to your account.
Payments
Patients should be prepared to pay their co-payments and deductibles at the time of their visit. We accept cash, Visa, Mastercard and Discover. Personal checks are not accepted.
What To Bring
- Any medical records concerning your condition
- A list of your current medications
- Photo ID
- Insurance card(s)
- Co-pay (if applicable)
Please carry an ID (driver’s license) and your insurance card with you to every appointment.
