To make your first appointment, please call 214-941-0198. Please let our receptionist know whether you have any special needs; we will make every effort to accommodate you.

Once you have your appointment, please download and complete the New Patient forms below. Completing the forms in advance will save you waiting time in the office.

Can't Keep Your Appointment?

Please notify us at least 24 hours in advance of your scheduled appointment if you know that you will not be able to come. This will allow us to schedule patients with urgent need of a visit.

Payments

Patients should be prepared to pay their co-payments and deductibles at the time of their visit. We accept cash, checks, Visa, Mastercard and Discover.

What To Bring

For your first appointment, please bring:

• Your completed New Patient forms
• Any medical records concerning your condition
• A list of your current medications

Please carry an ID (driver's license) and your insurance card with you to every appointment.
